2. How We May Use and Disclose Your PHI

For Treatment

We may use and disclose your PHI to provide, coordinate, and manage your healthcare. This includes sharing your information with the licensed physicians who conduct your telehealth consultations and the licensed compounding pharmacies that fulfill your prescriptions.

For Payment

We may use and disclose your PHI to process payments for services rendered, including billing, claims management, and collection activities.

For Healthcare Operations

We may use and disclose your PHI for operational purposes such as quality assessment, staff training, compliance audits, and business planning necessary to run our practice.

As Required by Law

We may disclose your PHI when required by federal, state, or local law, including reporting to public health authorities, responding to court orders or subpoenas, and reporting suspected abuse or neglect.

Health and Safety

We may disclose your PHI when necessary to prevent a serious threat to your health or safety or the health or safety of the public.

3. Uses and Disclosures Requiring Your Authorization

We will obtain your written authorization before using or disclosing your PHI for purposes not described in this Notice, including:

• Marketing communications (other than face-to-face communications and promotional gifts of nominal value)
• Sale of your PHI
• Most uses of psychotherapy notes, if applicable

You may revoke your authorization at any time by submitting a written request to us. Revocation will not affect any uses or disclosures made in reliance on your authorization prior to revocation.

4. Your Rights Regarding Your PHI

• Right to access: You have the right to inspect and obtain a copy of your PHI maintained in our records. We may charge a reasonable fee for copies.
• Right to amend: You may request that we amend your PHI if you believe it is inaccurate or incomplete. We may deny the request under certain circumstances but will provide a written explanation.
• Right to an accounting of disclosures: You may request a list of disclosures of your PHI that we have made, other than for treatment, payment, healthcare operations, or disclosures authorized by you.
• Right to request restrictions: You may request restrictions on certain uses and disclosures of your PHI. We are not required to agree to all requests but will comply with any request to restrict disclosures to a health plan for services you paid for in full out of pocket.
• Right to confidential communications: You may request that we communicate with you about your health information in a specific way or at a specific location (e.g., only by mail to a specific address).
• Right to a paper copy: You have the right to obtain a paper copy of this Notice upon request.
• Right to be notified of a breach: You have the right to be notified if there is a breach of your unsecured PHI.

5. Our Duties

We are required by law to maintain the privacy of your PHI, provide you with this Notice, and abide by the terms of this Notice currently in effect. We reserve the right to change this Notice at any time. Changes will be effective for all PHI we maintain. The revised Notice will be available on our Website and upon request.

6. Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
